Abdominal pain is a common Gastroenterology problem in Singapore. Dr Eric Wee, our Senior Consultant shares some points on how it is evaluated. There are many causes of abdominal pain. Most individuals will think about a stomach problem. It is often correct, but not always. A specialist needs to ascertain with absolute certainty where the problem lies, before instituting treatment. This year, we have made the following diagnoses in patients with abdominal pain: • Colon and pancreas cancers • H.pylori infection, gastritis and duodenal ulcers • Constipation • Infections of the gut • Anxiety • Gallstones and bile duct stones • Inflammatory bowel disease • Abdominal wall pain • Appendicitis • Diverticulitis Conditions like cancer must not be missed. To clinch the diagnosis, considerable time is spent understanding the patient’s problem. A physical examination is very important and telemedicine is not advisable. Tests that are required will depend on the location of the pain (upper, lower, left, right). For example: • Blood tests • Scans (ultrasound, CT or MRI scans) • Endoscopy (gastroscopy or colonoscopy) Further tests (allergy, food intolerance and small bowel tests) may be required, if the initial tests are inconclusive. Using this approach, almost all cases of abdominal pain can be diagnosed successfully.
