Your First Visit to Shuayb Dental Institute

Your first visit at Shuayb Dental Institute usually consists of a dental exam and a thorough analysis of your treatment options. In most cases, treatment can begin on the day of the initial consultation, unless of course, the patient requires a more intense treatment plan. Patients with a more involved medical history may require a second evaluation/appointment to begin treatment.

If you can please provide the following information for your consultation:

• List of all medications you are taking.
• Your dental insurance information.
• X-rays that were taken at your previous dental practice.

Important Information

It is required that any patient under the age of 18 is accompanied by a parent or guardian. If you have a medical condition such as high blood pressure, artificial heart valves, artificial joints, diabetes or if you are taking any medication please make the front office aware as soon as possible.

X-Rays

If you have had X-rays taken by your previous dentist please make a request that their practice sends them to our office via email. If you don’t have time to make the request ahead of time see if you can pick them up in person to bring to your appointment. Should you need an additional set of X-rays we can take them at our facility.

Financial Policy

At Shuayb Dental Institute we are committed to making payment as convenient as possible. We accept American Express, Master Card, Discover, Citi Financial and Care Credit. We promise to deliver the best care at the best price, that is why payment will always be due at the time your treatment is rendered unless arranged prior to receiving the service. Should you have any questions or concerns in reference to your account please call us.

It is important that patients remember they are 100% responsible for all fees accrued by Shuayb Dental Institute regardless of your dental insurance coverage.

Our office will send you monthly statements. In our experience insurance companies will respond within six weeks. If your statement does not reflect your insurance payment please give us a call. Remaining balances after your insurance payment have gone through will become your responsibility. Paying these statements as soon as you can is highly valued.
