What is an Advance Healthcare Directive?

In the Advance Healthcare Directive, you state your choices for healthcare or name someone to make those choices for you if you become unable to make decisions about your medical treatment.

The Virginia Health Information (VHI) administers the Advance Healthcare Directives Registry. This allows Virginia residents to create a free registry account where they can securely store their advance care planning documents.
