Dr. William A. Portuese is a double board-certified facial plastic surgeon with over three decades in practice. He is the founder and medical director of the Seattle Facial Plastic Surgery Center and the Portland Center for Facial Plastic Surgery in Portland, Oregon. A: You can get rhinoplasty at anytime, since the sculptra placed in your temples cheeks and forehead will have nothing to do with your nose.
A: A thorough internal examination of your nose is going to be required to make a determination about having a deviated septum. There are several other issues on the inside of the nose, such as valve collapse and turbinate hypertrophy, allergies, and chronic sinusitis that can cause obstruction. It's also important to know how much is left over on the inside of your nose for potential grafting purposes since you may have had a septoplasty nasal surgery. A rhinoplasty is performed to straighten the crooked nose.
A: Yes, indeed, you do have thin skin on the dorum, along with your nasal septum showing through, which is higher than your nasal bones. A revision rhinoplasty can accomplish shaving down the dorsal septum, which is composed of both bone and cartilage in that area. Osteotomies would be required to close any open roof, deformity, and harvesting temporalis fascia and inserting it over that area would camouflage that area as well to help with thin skin present.
A: A closed rhinoplasty approach can accomplish shaving down the dorsal hump, narrowing the bridge line, and refining the nasal tip with all the incisions placed on the inside of the nose.
A: If you have a deviated nasal septum which is symptomatic, then a septoplasty can be performed for medical necessity and billed to the patient's medical insurance. Co-pays and deductibles will apply. Straightening the outside of the nose requires a cosmetic rhinoplasty, which must be paid for by the patient. Both procedures can be done simultaneously under one anesthesia with one recovery.
