Dr. William A. Portuese is a double board-certified facial plastic surgeon with over three decades in practice. He is the founder and medical director of the Seattle Facial Plastic Surgery Center and the Portland Center for Facial Plastic Surgery in Portland, Oregon. Widely considered one of the premier facial plastic surgeons in the Pacific Northwest, Dr. Portuese specializes in all areas of aesthetic facial surgery, including facelifts, neck lifts and liposuction, blepharoplasty, rhinoplasty (including revision rhinoplasty), and forehead lifts, as well as cheek and chin implants. “Because of my double board certification, I can extend a high level of expertise, mastery, and professionalism to my patients,” says Dr. Portuese. Dr. Portuese earned his Bachelor of Science degree and his medical degree from Oklahoma State University. He completed an internship in categorical otolaryngology–general surgery and residency in otolaryngology–head and neck surgery and facial plastic and reconstructive surgery at Louisiana State University Hospital, followed by a year-long fellowship in facial plastic and reconstructive surgery at the renowned Lasky Clinic in Beverly Hills, California. “Early in my career, I sought out the best of the best surgeons doing facial cosmetic surgery, specifically facelifts, eyelid lifts, and rhinoplasty,” says Dr. Portuese. “I observed and assisted these master surgeons performing their craft, and that expertise is what I bring to my practices in Seattle and Portland.” Dr. Portuese is board-certified by both the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology–Head and Neck Surgery. He is a clinical instructor in otolaryngology at the University of Washington, where he teaches residents the latest techniques in cosmetic rejuvenation procedures. Q: Septorhinoplaaty and insurance - what is the likelihood of it being straightened as part of the procedure? (Photo)

A: If you have a deviated nasal septum which is symptomatic, then a septoplasty can be performed for medical necessity and billed to the patient's medical insurance. Co-pays and deductibles will apply. Straightening the outside of the nose requires a cosmetic rhinoplasty, which must be paid for by the patient. Both procedures can be done simultaneously under one anesthesia with one recovery.

Q: Local Anesthesia Toxicity?

A: We've never seen local anesthesia toxicity performing facelifts for over 25 years. You can perform a mini lift under local anesthesia, but trying to perform an entire comprehensive lower face and neck lift, It's best to go under general anesthesia for patient safety and comfort.

A: A full set of facial photographs from all angles are required to make a determination about being a candidate for a rhinoplasty procedure. In our practice, we perform closed rhinoplasty with all the incisions placed on the inside of the nose. No external incisions, and no painful packing either. An in person examination is going to be required to make a determination about how best to proceed regarding your congestion. Best to try multiple different medical management options prior to any functional nasal surgery. A CAT scan of your sinuses is going to be required to determine issues regarding chronic sinusitis.

Q: Should I consider a genioplasty/chin augmentation? (Photo)

A: A genioplasty is performed by an oral surgeon under general anesthesia in a hospital setting with an overnight stay in the hospital and is much more invasive. As long as your teeth fit together properly, you can undergo a simple chin implant placement under local anesthesia As an outpatient procedure, which is much less invasive.

Q: 25 year old chin implant that I hate, what are options?

A: Much more information is needed, such as a full set of facial photograph from all angles in a chin neutral position. It would also be helpful to see what your photographs of you looked like prior to the chin implant placement, Especially from the side profile to understand what your pre-existing anatomy was. First option would be to simply remove the implant and go back to your original anatomy, or remove the current implant and place a smaller one.
