- Any X-rays taken by your previous dentist
- A list of all medications you are currently taking
- Any current medical concerns or conditions you may have
- Your dental insurance information, if you are covered and will be paying through insurance.
- If you’d like us to obtain a copy of your X-rays from your last dentist, please request that they be forwarded to our office before your appointment.
